Application Packet and Instructions
If you wish to apply:
Please print and complete the Enrollment Form.
The enrollment form must be received by March 1st of the enrollment year.
South Hingham residents must apply by January 1, in order to have a preference.
Please mail the completed enrollment form to:

Please include:
- · A copy of your child's birth certificate;
- · A check for $45 to cover the application fee.
The $45 fee is refundable only if your child is NOT placed in one of our programs. Your check will be returned to you after the lottery. If you are enrolling more than one child the fee remains $45.00 as it is a per family charge. Please include a separate Application Form for each child.
Please indicate on the enrollment card, using 1,2,3, only the classes YOU WOULD ACCEPT. Please be aware that choosing only one program on the enrollment card does not give your child an advantage in any class. Children are placed in classes in the order that the cards are pulled. If you do not get your first choice you are automatically waitlisted for that class and placed in your second choice if there are openings. Your child may be placed in your third choice if openings are not available in either your first or second choices. All children not enrolled are waitlisted.
When we receive your application we will email you a confirmation of receipt of your application along with a packet with more information.
Your child must be three years old by August 31, 2011, in order to be considered for placement in our program.
